The document indicates that Mackie suffered a serious head injury after falling on September 11 at the home of his longtime design director, Joseph McFate. He passed away three days later, on September 14, at a hospital in Palm Springs.
A representative for Mackie confirmed that he had been staying in McFate's guest house and that a healthcare professional was present at the time of the fall. The rep also noted that Mackie had been dealing with pneumonia both prior to and during his hospitalization.
Following his death, Mackie was cremated, and his ashes were scattered at sea off the coast of San Diego County.
Mackie, who was 87, was renowned as one of Hollywood's prominent costume designers, having dressed icons such as Cher, Elton John, and Carol Burnett throughout his illustrious career.
